Issue:

Veteran As wife called the Dorn VA Medical Center Release of Information Office (ROI) to inform them that her husband received copies of a three page medical report by mail, which was intended for Veteran B. The report was inadvertently mailed to Veteran A by the ROI Office. The ROI Office reprocessed Veteran B's request, and a copy of the tree page report was mailed to the correct address. A formal letter has been prepared for the Acting Medical Center Directors signature, requesting that Veteran A return the three page report to the Dorn VAMC. The information disclosed was Veteran B's full name, full social security number, address, name of provider, and the personal health information. Update: 11/04/11:Veteran B will receive a letter offering credit protection services.

Outcome:

Staff is being re-educated on the need to safeguard patient information.
