Issue:
Based on staff interview and record review, the facility failed to maintain the privacy and confidentiality for one patient (Patient A) when an employee included Patient A's name on discharge instructions for another patient.Findings:An interview was conducted on 4/16/12 at 10:00 AM with the facility's Health Information Officer (HIO). The HIO stated that on 3/15/12, it was reported to him that an employee inadvertently put another patient's name on Patient A's discharge instructions. The HIO further stated that the breach was identified when Patient A noticed that although discharge instructions were correct, the wrong name was on her discharge instructions. A review of the information that was on the discharge instructions disclosed the following personal health information (PHI):Patient nameThe HIO further stated that Patient A was notified of the breach occurrence. The employee was educated and disciplined.
