Issue:

On February 23, 2015, Veteran A telephonically requested from Pharmacy Service that his medications be sent via UPS to his work address. On March 2, 2015, Veteran B, who has a similar name, called the Pharmacy to inquire about the status of his medications that were being mailed. Upon Pharmacy Service checking UPS tracking, it was then found by Pharmacy Service that they inadvertently mailed the package of medication of Veteran B to Veteran A. Veteran Bs information that was disclosed on the medication package included his Full name, address, and the type of medication. Veteran A has been contacted by Pharmacy Service to arrange the return of the medications.

Outcome:

03/05/15: The Incident Resolution Service Team has determined that Patient B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
