Issue:
The back page of a medication profile from Veteran A was inadvertently mailed to Veteran B. Veteran B called the pharmacy to inform them immediately upon discovery and had destroyed the page prior to calling. The information at risk included Veteran A's name, address, and medication information. Update: 08/16/12:Veteran A will be sent a notification letter.
Outcome:
The medication sheet was retrieved and destroyed. Procedures were reviewed with staff.
