Issue:
Veteran A received correct medication with incorrect medication labeling of Veteran B. The Veterans have the same first and last name - but middle name is different. The labeling has Veteran B's name and address and was promptly returned to the VA by Veteran A. Update: 09/08/11:Veteran B will be sent a notification letter.
Outcome:
Education will be provided to the offending employee by the pharmacy chief regarding checking paperwork before it is placed in bags for mailing.
