Issue:

A ticket was entered on 12/07/11 (SPE 69395) for an employee leaving her own pay stubs in a hallway in Bldg 5 of Brecksville. Reviewing the evidence again today, there were 2 other employees information in the packet. Since the data included other employees this is being reported. Data was left unsecured in a vacant building (in the hallway). The building at the time was unsecured for an unknown timeframe. A patient may have walked through the area. Staff are unable to determine if anything is missing. These two slips were in the same packet reported in SPE 69395 but they were not discovered at the time. The employee is an HR employee so the pay slips are probably appropriate to have in her possession, but they were left them behind. Update: 01/10/12:The two additional employees will be sent letters offering credit protection services due to full name and full SSN being exposed.

Outcome:

Credit Monitoring offered and sent. Case turned over to Supervisor of employee who counseled the employee on leaving PHI unsecured.
