Issue:
A progress note belonging to Veteran A was inappropriately entered into Veteran B's medical records. Veteran B obtained a copy of their medical record to include a five page progress note that belonging to Veteran A. Unfortunately, Veteran B and Veteran A are currently enrolled in the same group therapy and Veteran B could determine the identity of Veteran A.
Outcome:
05/13/13: Veteran A will receive a notification letter.
