Issue:
Patient A contacted the medical center after he received his prescription package. The package had been received with all of the prescription documents stapled to the outside of the package. The patient's name, address and type of medication were disclosed while in transit with the postal service. Great Lakes Consolidated Mail Outpatient Pharmacy (CMOP) investigation concludes that the package had not been sealed properly by the CMOP packer. The employee will be counseled and retrained in proper packing procedures. Update: 08/17/11:Patient A will be sent a notification letter.NOTE: There were a total of # Mis-Mailed CMOP incidents out of # total packages (# total prescriptions) mailed out for this reporting period. Because of repetition, the other # are not included in this report, but are included in the "Mis-Mailed CMOP Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter.
