Issue:
A VISTA appointment list printed for Veteran A was inappropriately given to Veteran B in error. It contained Veteran B's name, full SSN and appointment information. Update: 02/21/13:Veteran A will be sent a letter offering credit protection services.NOTE: There were a total of 14 Mis-Handling incidents this reporting period. Because of repetition, the other 13 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Education has been provided to the employee responsible for the error. Credit Monitoring offer/notification letter has been sent to the impacted Veteran.
