Issue:
An eligibility clerk had a stack of 5 Veteran applications on her desk. She turned the papers over so that the Veteran, whom she was meeting with, could not see names and other information. When she was finished meeting with the Veteran, she noticed that her stack of applications were gone. She believes that the Veteran accidentally picked up this stack, while picking up forms that belonged to him. The eligibility clerk is working to contact this Veteran to have him return the 5 applications. Update: 09/06/11:Five Veterans will be sent letters offering credit protection services due to name and SSN being exposed.
Outcome:
Employee was reprimanded. Copy of Credit Monitoring letters are attached.
