Issue:

Veteran A arrived at the front desk to check in, it was around 3:42pm, he stated that the clerk was rude to him, handed him a printout containing Veteran B's info to include full name, full SSN DOB, home address, and Veterans benefits info. Veteran A proceeded to Mental Health and turned in this paperwork to a Social Worker to safeguard. Veteran A walked from one clinic area to another while never leaving the facility. The Social Worker stated it was approximately a 5-8 minute walk, Veteran B's information was out of VA control for a short period of time. The Social Worker has forward Veteran B paperwork to PO for proper handling. Update: 10/15/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

MAS Supervisor has informed me that she will be conducting training for all MAS staff on the importance of reviewing all documentation prior to handling it to the patient, also will ensure that employees are aware of what location they send their print jobs to. Protection of personal health information will be discussed at all monthly meetings. Also MAS Supervisor stated that any employee not able to attend training on Thursday that individual training will be conducted along with a sign-in sheet provided for documentation. Finally effort will be taken to identify the actually individual who was rude and responsible for the situation to discuss this situation and address it appropriately. Do to the fact that Veteran A had procession for such a short time and never left the facility also since he personally handed it to the MH Provider and requested that he take the form and dispose of it properly, I feel that no breach has taken place and request this ticket to be closed.
