Issue:
Based on interview and record review, the facility failed to ensure the PHI (protected health information) of Patient A remained confidential. This failure resulted in the unauthorized disclosure of Patient A's PHI to an unintended recipient, Patient B.Findings:An interview was conducted with the Director of Health Information Management (DHIM), on April 30, 2014, at 10:05 a.m. The DHIM stated Patients A and B were both admitted to the third floor nursing unit. Both patients had physician orders for Zosyn (an antibiotic). The registered nurse printed two labels for the antibiotic and placed the label for Patient A's antibiotic on the intravenous bag met for Patient B.The DHIM stated both intravenous bags were labeled with the same medication by the facility's pharmacy and were part of the nursing units stock medication supply.The DHIM further stated a member of Patient B's family noticed the incorrect label on the patient's intravenous bag and notified the nurse. A review of the identifying label which was placed on Patient B's intravenous bag was conducted. The label reflected Patient A's name, account number and diagnosis.
