Issue:

A CBOC sent 6 prescriptions for 5 Veterans and a list via USPS to our Fort Wayne facility. The package was received in the mail room and taken to the pharmacy. The envelope was opened in the pharmacy and the list was removed. When checking for the 6 prescriptions, they were not found. Pharmacy has checked the shred bins, the trash, and all work areas. The prescriptions had the Veterans first and last name, last 4 of the SSN, address, provider name and DEA number, medication name/directions/dosage, and the prescription number. Update: 10/05/12:Five Veterans will receive a notification letter dut to medication information being lost.

Outcome:

The pharmacy has instituted the following:Prescriptions from CBOCs will now be sent by UPS. This will allow for them to be delivered directly to the pharmacy instead of going through the mail room.These envelopes are to go to the pharmacy vault for distribution to the appropriate person. Process has been sent to all pharmacy staff. SOP is being revised and will be communicated to all pharmacy staff. CBOCs will be converting to 8 __ x11 sheets for control prescriptions. This will decrease the possibility of the smaller prescriptions being missed in the envelope.
