A form containing a copy of drivers' licenses for seven employees has been misplaced by clerical staff. A search for the form has not revealed any results. Update: 06/21/11:Seven (7) Employees will received letters offering credit protection services due to full name and DOB being exposed.

Responsible employee to receive appropriate administrative action and additional training. Referred to service chief.
