Issue:
Veteran A was given Veteran B's appointment list at an appointment earlier this afternoon. Veteran A reported to another appointment with the appointment list. The clinic realized Veteran A had Veteran B's appointment list. The clinic regained custody of the document and notified the Privacy Office.
Outcome:
04/17/14: The Incident Resolution Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
