Issue:

A Patient's wife called the Director's office to inform the medical center that she and her husband came in to Release of Information (ROI) department to get copies of his medical records. When they got home and opened the envelope, they say the medical records were for another Veteran. The protected health information (PHI) included full name, full SSN, medical information and date of birth. They stated they were in the Atlanta VAMC for a procedure and are due to fly back to Alabama where they live. They will mail the records back to this Privacy Officer (PO). Update: 11/09/12:Patient B will be sent a letter offering credit protection services.

Outcome:

Supervisor has counseled staff and will evaluate for disciplinary recommendations. Staff are up-to-date on training.
