Issue:

On 08/18/14, Veteran A called and contacted the Fee Basis Section to report that when he received a letter in the mail from the Fee Basis Section, he found the paperwork in the envelope that belonged to Veteran B. It was determined that this was a Fee Basis Claim Rejection Letter and contained only Veteran's B mailing address and an assigned claim number within Fee Basis. No other identifying information on Veteran B is contained in the letter. Arrangements were made for Veteran A to return the mis-mailed letter to the Privacy Officer.

Outcome:

08/18/14: The Incident Resolution Service Team has determined that Veteran B will be sent a general notification letter.
