Issue:
On, 03/09/15, the Privacy Officer (PO) reported a privacy incident that she received by inner office mail. On 01/27/15, Veteran B presented to the lab with a lab draw order for Veteran A. The lab recovered the order and requested an appropriate order from the provider. Documentation contained Veteran A's name, full SSN, date of birth, age, and lab tests. A Report of Contact (ROC) states this is a frequent occurrence from one specific outpatient clinic. Additional notification and investigation, to ensue.
Outcome:
03/10/15: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
