Issue:
Medical records were disclosed to another Veteran with the same first and last name, different middle name. Veteran A who received wrong information hand delivered records to Veteran B whom the records actually belonged to. Update: 10/18/11:Due to full SSN and medical information being exposed, Veteran B will be sent a letter offering credit protection services.
Outcome:
PO states incorrect lab requisition form was mailed to the wrong patient. Patient who received the discharge instructions returned them to Togus. They were mailed to the correct patient. Employee who performed the error was notified and apologized. Notified to be very cautious of this in the future.
