Issue:

A patient list dated 06/07/11 for a VA program was left in a conference room. The list contained the names and last four digits of the SSN for 16 patients. This conference room is used for a variety of purposes. It is usually occupied by employees but programs for Veterans/patients may also be held here. Update: 06/21/11:The sixteen (16) patients will be sent notification letters.NOTE: There were a total of 15 Mis-Handling incidents this reporting period. Because of repetition, the other 14 are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Staff reminded to safeguard documents with PII and, if possible not to ask for any portion of SSN
