Issue:

When Veteran A checked in for his regular appointment, he asked for a copy of his Dr's Notes and lab results. In her haste to assemble the paperwork and provide it to the Veteran, the program support assistant also scooped up other paperwork laying atop the counter. The other paperwork consisted of a My HealtheVet application form (10-5345a-MHV) and photocopy of VIC Card from Veteran B. Veteran A took the paperwork and returned to his seat in the waiting room. After 15 minutes, he discovered the clerk's mistake and returned the application form and photo to the clerk and proceeded to wait for his name to be called. At no time did the Veteran depart the waiting area, nor appear to copy any information from the form. Update: 07/27/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

The program support assistant has been cautioned by her supervisor to exercise more diligence and to avoid rushing.
