Issue:
HR Specialist was sending financial (bank account numbers) information as well as full SSN among other information to Employee A but instead it went to Employee B with the same name. It is unknown at this time if Employee A sent the information using PKI and no response has been received from Employee B. Update: 02/18/11:Internal email, included SSN and bank information sent to wrong person. Employee A will be sent a letter offering credit protection services.
Outcome:
Supervisors have met with the employee and developed a new process to notify participants of their service period ending. They will no longer attach any forms in an email message. The employee must come to the office to pick-up the form. If the employee is unable to pick-up the form, they will forward a blank form for the employee to complete.Also, copy of redacted credit monitoring letter is attached so this ticket can now be closed.
