Issue:

Based on interview and hospital document review, the hospital failed to prevent the disclosure of Patient A's protected health information (PHI) to unauthorized individuals.Findings:Review of hospital documentation showed on 4/20/14, Patient A, a staff member, was taken to the emergency department then admitted to the hospital after being violently attacked by a visitor to the hospital. Review of the hospital's investigation showed an audit of Patient A's medical was completed. A total of six staff members, who had no part in the care and treatment of Patient A, accessed the patient's electronic medical record (EMR). Interviews with the six staff identified showed they did not deny their unauthorized access. Patient A's disclosed PHI included the care and results of treatment received in the emergency department and hospitalization after the violent attack occurred on 4/20/14.On 7/30/14 at 1550 hours, an informal telephone conference with the Compliance Officer confirmed the breach occurred as documented.
