Issue:

An amendment request document is missing within the service. The provider said he provided it to his secretaries, but they do not have the documents. The documents contained the patient's name, address, date of birth, full SSN and protected health information. Update: 08/02/12:The paperwork is a progress note. A patient requested an amendment request. Privacy sent the paperwork to the Provider to review. The provider said that he gave the paperwork to the secretaries, but they don't recall him giving it to them. Privacy is interviewing staff and we will double check the paper trail to see if it can be located. When Privacy didn't receive the documents, it was reported.One Veteran will be sent a letter offering credit protection services.

Outcome:

Staff was interviewed and reminded how to safeguard VA sensitive information so this does not happen again.
