Issue:

Veteran A's physical address was changed in VistA, and medication was subsequently mailed to the incorrect address. Veteran A called asking where his medication was. The person who received the medication by mistake also called in about it. It is unknown at this time if the person who received the medication was a Veteran or not. The ISO had the Programmer Application Specialist perform an inquiry on the Patient record and spoke to the clerk who changed his address on 2/22/11. She said she received a call from the patient to change their address, and she did as was requested. I have a call into pharmacy to obtain more details. Update: 03/08/11:Veteran A's data that was sent to the other individual included his name, address, and medications. Veteran A will be sent a letter of notification.

Outcome:

Review of process for address changes was performed and education provided to personnel who perform that function. Notification letter sent to patient.
