Issue:

The following incident was reported to the Privacy Officer, Nashville Campus: On April 5, 2007, a Patient was enrolled into a joint research study between VA Tennessee Valley Healthcare System (VA TVHS) and its affiliate. During this timeframe, the consent document contained the consent portion, which authorized participation in the study, and the Healthcare Portability and Accountability Act (HIPAA) portion, which authorized the use of the patients personal identifiable information. During a recent audit of this protocol, the Research Compliance Officer (RCO) found the HIPAA portion of the consent was missing from this Patients file. Despite exhaustive efforts, the research team was unable to locate the missing HIPAA.

Outcome:

01/08/15: The Incident Resolution Service Team has determined that the Patient will be sent a letter offering credit protection services.
