Issue:

A VA provider mistakenly gave a copy of an appointment list that contained the names, full SSN's, and the reason for the appointment to a Veteran. The list had 10 Veterans' names. The Veteran noticed he had the list and returned them to the clerk prior to checking out of the clinic. The appointment list never left the clinic and the patient had the document a total of 2 to 3 minutes prior to returning it to the clerk. Update: 02/03/12:Ten (10) Veterans will be sent letters offering credit protection services.

Outcome:

Notification letters mailed on 3/9/12, re-educated provider on safeguarding PHI.
