Issue:
Veteran A reported through the national VA Patient Advocate's office stating: "I received health items (catheters) in the mail that were supposed to go to another veteran with the same first and last name. What do I do?" There are 20 Veterans in the Portland VA Medical Record with this Veteran's same first and last name. Pharmacy is contacting the Veteran to determine if the mailing was a result of something mis-ordered for him, or if another Veteran's order and documents were mailed to the wrong address. Update: 08/08/12:Veteran B will receive a HIPAA letter of notification.
Outcome:
The Pharmacy office mailing packages is working with the mail room handling their packages to ensure addresses are not changed when supplies are put in new containers for shipping.
