Issue:

Today, a Veteran/Employee (Medical Support Assistant, Business Office, Nashville Campus) obtained a copy of a Sensitive Patient Access Report (SPAR) for the period of March 1, 2013 through October 17, 2013. Veteran/Employee found that his co-worker, a Medical Support Assistant, Business Office, Nashville Campus inappropriately accessed his records on seven (7) occasions during this timeframe. Per the Veteran/Employee, his co-worker is not involved in his care and should not have accessed his records. Veteran/Employees personal information includes: full name, full social security number, date of birth, address, contact information, medical history/diagnoses, lab results, x-ray reports, and medications.

Outcome:

10/17/13: The employee will be sent a letter offering credit protection services.
