Issue:

A VA Nurse in Primary Care had a clipboard on corner of her desk that had handwritten notes by the nurse regarding employee contact information on two new employees. A third page was a Workers' Compensation Log that had names only of four employees plus the lab results of a positive TB test on one of the four employees. Update: 07/01/11:The nurse did have authority to access this information and she did not leave it unattended, however when a Veteran picked up his own papers, he accidentally picked up the clip board. The Veteran was in posession of the clipboard for four days. Two employees will receive an offer for credit protection services.

Outcome:

Employee reminded to safeguard information in office and on desk. She has clear understanding of significance of event.
