Issue:

A VA doctor wrote a total of three (3) prescriptions (two (2) for Vetertan A and one (1) for Veteran B) and gave them to the MAS clerk. The two prescriptions for Veteran A were to be mailed, the one prescription for Veteran B was to be sent to the pharmacy for pick-up by Veteran B. The VA doctor received a call from the pharmacy a few days later concerning Veteran B who needed a new prescription. VA doctor found out that Veteran B's prescription was mailed to Veteran A. Veteran B's full name, full SSN, medication, and dosage was all mailed to Veteran A. VA has contacted Veteran A to return the medication to the VAMC. Update: 03/20/12:Veteran B will be sent a letter offering credit protection services due to full name and full SSN being disclosed.

Outcome:

Credit-monitoring letter sent, and the supervisor of the responsible employee has taken appropriate supervisory action.
