Based on interview and record, the hospital failed to ensure that Patient 1's medical information was kept confidential from anyone not authorized access.Findings:An interview with the emergency department clinical lead (EDCL) was conducted on 2/28/13 at 3:05 P.M. The EDCL stated that a registered nurse was discharging Patient 2 but instead gave Patient 1's discharge documents to Patient 2. The discharge document that was inadvertently given to Patient 2 indicated Patient 1's name, date of birth, medical record number, chief complaint, laboratory tests performed, and diagnostic tests performed. The EDCL stated that the registered nurse should have checked the documents to ensure that the document was being given to the correct patient.
