Issue:

Patient provided a written statement to the Birmingham VA Medical Center (BVAMC) Privacy Officer (PO) that he left his non-VA medical records at the BVAMC Blue Clinic several months ago. Patient further states the receptionist at the BVAMC Blue Clinic assured him that his records would be given to his primary care physician and then be sent to medical records for scanning. Patient had to go to BVAMC emergency room today and when his primary care physician's nurse reviewed his records, he was told the non-VA medical records were not on file. Patient then went to BVAMC Business Management Service (BMS) and discussed the situation with the Chief, BMS, who verified that the non-VA medical records were not scanned into patient's VA records. Patient feels all of his personal information, including social security number and private medical information are in jeopardy. However, this cannot be confirmed as a copy of patient's non-VA medical records are not available to review. The patient is requesting credit monitoring. Update: 11/14/12:The patient will receive a letter offering credit protection services.

Outcome:

Chief, Business Management Service (BMS), Birmingham VA Medical Center (BVAMC), discussed the missing records with the physician and staff at the BVAMC Blue Clinic. All areas were searched for the missing records, but said records were not located. A credit monitoring letter has been mailed to the patient.
