Issue:

Employee misfaxed documents on a Veteran. Employee was notified by the person in the general public who received the fax at his home. Employee told person to destroy the documents. The Privacy Officer (PO) contacted the person and they had not destroyed the documents yet. The PO asked the person to mail them back to me and they said they would. The information at risk included the Veteran's name, address, date of birth, full SSN and protected health information (PHI) Update: 05/05/12:The Veteran will receive a letter offering credit protection services.06/08/12:The Privacy Officer (PO) requested that the ticket be re-opened. There was a problem with a page of the redacted letter. New redacted letter uploaded and original redacted letter removed by Remedy Administrator.

Outcome:

Education was provided to the staff regarding faxing policies and procedures that is outlined in the CM 00-36. It was completed on 05/09/2012. The ticket information was forwarded to Human Resources for the appropriate administrative action.
