Issue:
Transitional Living Center (TLC) Activity Sheet was discovered in the TLC activity room by a VA employee. The paperwork was immediately reported to the Privacy Office and the paperwork was given to the TLC supervisor. The supervisor had the paperwork destroyed prior to this investigation. A copy of this paperwork was given to the Privacy Office for reporting purposes. The paperwork contained the patients' full name, diagnosis and date of birth Update: 01/10/11:The PO states there are twenty - seven (27) names on the paperwork. Documents was left less than 24 hours. The room can be accessed by anyone visiting the TLC. Additional information on the Activity Sheet included the NOK and their contact number. Twenty-seven (27) Veterans will receive a letter offering credit protection services.
Outcome:
Nursing Manager briefed all employees of their responsibilities for protecting sensitive information. Credit protection letters sent out this date. Recommend incident be closed.
