Issue:

A Veteran had an appointment with his provider on 07/19/11. At the end of the appointment, the provider handed the Veteran some information. When the Veteran looked at the information at home, he had a list of the provider's patients for the day the Veteran was seen. The Veteran called the Privacy Officer (PO) on 07/21/11. The Veteran will mail back the information in the envelope the PO is sending to him. Once the PO has the information the PO will update the ticket. Update: 07/22/11:The Veteran stated that the earliest the list could be mailed is 07/22/11. The Privacy Officer is unable to identify which patients are on the list until it is received at the Medical Center.07/27/11:The PO received the paperwork in the mail. It contained a list of appointments for ten Veterans which included the Veterans full name and full SSN. Ten Veterans will receive a letter offering credit protection services.

Outcome:

The supervisor of the provider has provided additional education on securing information with PHI/PII. The provider has also been told to retake his privacy policy web training.
