Issue:
A Fax sent to the correct number but an incorrect area code. The recipient immediately contacted VA. The employee was identified. The patient and his wife still on ward. The patient was immediately notified. The recipient agreed to return the materials and was met at an off station site of his selection to return documents. No copies were made or information distributed by recipient. The documents contained the name and diagnosis of the patient. Update: 09/23/11:The patient will receive a letter of notification.

Reviewed with staff faxing process and procedures.
