Issue:

On 12/30/11, the Administrative Officer for Compensation and Pension contacted VBA to check on the receipt of records shipped on 12/16/11. VBA did not receive the three boxes and 1 package containing 17 Veteran medical records shipped via UPS. VA reported the missing records to UPS on 12/30/11. UPS has begun an investigation and has eight days to complete the search. The facility will be notified via email of the investigation results. Update: 01/23/12:The boxes of claim files sent to VBA have not been found by UPS. The Administrative Officer for Compensation and Pension has gone to the UPS national level for help in locating the boxes. UPS is still looking however at this late of date they are unsure if boxes can be located. The personally identifiable information (PII)I included full name, full SSN, and date of birth.01/24/12:After further review it was determined that 24 Veterans were affected. Even though the VA is not at fault, the 24 Veterans will receive a letter offering credit protection services.

Outcome:

Was not VA fault but we will offer credit protection. Credit monitoring letter uploaded.
