Issue:

A Patient Services Assistant (PSA), Nashville Campus, contacted the Privacy Officer (PO), Nashville Campus, to report the following incident: A PSA, Nashville Campus, received an unencrypted email from a Veterans daughter in which she requested assistance with enrolling her father into the VA Tennessee Valley Healthcare System (VA TVHS). The PSA, Nashville Campus, responded to the daughter via unencrypted email and inadvertently sent the information to an unintended email address. The PSA, Nashville Campus, learned of the error when he was contacted by one (1) of the six (6) unintended recipients of the email. The PSA, Nashville Campus, attempted to recall the message but was not successful. The email contained the Veterans full name, full social security number, date of birth, place of birth, and mothers maiden name

Outcome:

07/11/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
