Issue:

A volunteer employee admitted to an employee that the volunteer accessed the file of the employee and asked the employee if the employee would be working on their birthday. The employee asked how the volunteer employee knew the birthday. The volunteer allegedly admitted that the volunteer went into the emplyee's paper file. In the file, along with the birthdate, is other Personally Identifiable Information (PII), as well as Protected Health Information )(PHI) such as: SSN, Home Address, phone number, medical information and emergency contact information. Update: 11/08/12:One employee will be sent a letter offering credit protection services.
