Issue:

On August 12, 2014, the Supervisor, Release of Information (ROI), was contacted by an Employee, Chattanooga State Community College, who stated the Chattanooga Community Based Outpatient Clinic (COPC) inadvertently sent her a Veteran's information. The Employee agreed to fax the information to the Supervisor, ROI. At this time, it is unknown what specific personal identifiable information was released. Thus, the Privacy Officer (PO) is entering this ticket as a complaint but will update to an incident if warranted.

Outcome:

08/19/14: PO update -- PO received the paperwork back from the Community College where the faxed too. It was found that the paperwork contained the Full name, Last four of the SSN, Date of Birth, address and a diagnosis. 08/19/14: 08/19/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
