Issue:

A Veteran provided copies of his x-rays and MRI images to be used in an orthopedic consult to obtain a second opinion on a medical condition. A review of the patients CPRS notes indicates that the images were viewed by the provider in orthopedics and the provider annotated that the images should be scanned into the records or that new images should be obtained. The images were never scanned into the record and an attempt to find the images has produced a negative result. The Veteran has filed a complaint with the facility director on several issues including why his images were lost. It is unclear if the images were lost or destroyed and the Veteran is already aware that the facility cannot locate the images. Update: 04/23/13:Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being misplaced and unable to be found.

Outcome:

Training was conducted with the staff involved to emphasize the importance of protecting Veterans medical records and the proper procedures for routing physical records to be scanned into the electronic record
