Issue:

A clipboard containing personally identifiable information (PII) and protected health information (PHI) on 4 different Veterans, was found in an unlocked, unattended physicians' team room this morning. The clipboard was left in this team room, after clinic, overnight and it is unknown if the room had been secured after 4:30 PM. Update: 04/27/11:Four (4) Veterans will receive a letter offering credit protection services.

Outcome:

4/29/11 - The privacy issue was discussed in detail with the employee and counseling was given. The service staff was reminded of the importance of patient record privacy issues, specifically, no patient information should be left unattended in this room unless it is secured in a locked cabinet or desk drawer and the room should be kept locked after hours.4 Credit monitoring letters were sent and a redacted copy was e-mailed to the VAIRCT mailbox.Please close this ticket.
