Employee A entered record of Employee/Veteran B. Employee A stated that it was believed the SSN of the employee/Veteran B was one of the SSNs employee A processed. Employee A also stated that email from Employee C regarding the death of employee/Veteran B but does not recall why the patient record was accessed. Update: 08/17/12:This is still being investigated by the facility.08/30/12:Employee/Veteran B's next of kin will be sent a letter.
