Issue:
A VA employee handed Veteran A radiology information belonging to Veteran B. The information included full SSN, full name, date of birth, and a copy of the report and the radiology exams (X-rays). The information was reported to the Privacy Officer (PO) as being returned on 02/25/13. Update: 02/25/13:Due to full SSN and medical information being exposed, Veteran B will be sent a letter offering credit protection services.NOTE: There were a total of # Mis-Handling incidents this reporting period. Because of repetition, the other # are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
The employee was given the appropriate disciplinary action. They were also asked to make sure they have taken their Privacy Training.
