Issue:

Today, 06/13/13, the Pharmacy Supervisor reports to Privacy Officer (PO) that yesterday evening his staff determined that a medication and educational documents for Veteran A were provided to Veteran B on 06/11/13. This was discovered late yesterday, 06/12/13, when Veteran A came to pick up his vaulted medication. Veteran B also had a vaulted medication plus another medication. Upon discovery, Veteran As medication was freshly prepared and dispensed to him. Pharmacy contacted Veteran B who had not yet taken Veteran As medication. This will be returned to the facility, coordinated with his appointment on Friday, 06/14/13. Both Veterans had vaulted medications and same common last name. Other root cause issues are being investigated. Additional notification and investigation to ensue.

Outcome:

06/13/13: Veteran A will be sent a notification letter.
