Veteran A telephoned Truman VAMC to report that there was other Veterans' protected health information in his USPS bubble mailer . The Veteran explained his concern for the receipt of this information, and that he would be "glad to return it" so that "no one would get in trouble." Update: 07/25/11:Twenty (20) Veterans will be sent letters offering credit protection services.07/26/11:The Privacy Officer (PO) spoke with Veteran receiving documents, he will be mailing documents back to the facility. It appears majority of documents were lab results (PHI) and other documents were PII. The counts may change.08/23/11:New count is 23 Veterans, therefore 23 Veterans will receive an offer of CPS.

Outcome:

Appropriate action taken by HR and service line. Credit monitoring letters mailed. Redacted letter attached.
