Issue:

Patient A was discharged from the Emergency Department on 12/13/11 and was given the discharge paperwork for Patient B by a resident. Patient A realized this when he went back to his inpatient room. Patient A's roommate took the discharge paperwork and stated he tried to turn them into a nurse on 4B. Patient A's roommate states the nurse refused to take the paperwork so the roommate called the VA Police to report the incident. The VA Police arrived and took statements from all involved. The VA Police took the discharge paperwork from the roommate and turned it into the Administrative Officer of the Day (AOD) who then put the paperwork in the proper filing system. Update: 12/15/11:Patient discharge paperwork included full SSN and Name. It also listed discharge directions and current medication. therefore 1 patient will receive a letter offering CPS.

Outcome:

Doctor was verbally counseled by the Chief of the Emergency Department.
