Issue:

Based on interview and record review, the facility failed to prevent unauthorized access to a patient's (Patient A) PHI (Protected Health Information) when a LVN (Licensed Vocational Nurse) gave Patient A's prescription to another patient.Findings: On April 17, 2013, a record review was conducted. The record indicated on March 20, 2013, Patient A was seen in the clinic and a prescription for an antibiotic was written. According to the records, the clinic's LVN gave the prescription to Patient B. The prescription included Patient A's name, date of birth, address, phone number, medical record number, and facility account number.During an interview with the facility's executive secretary, she stated, "Patient A's prescription was given to another patient. When he got to his car he realized the prescription wasn't his and brought it back to the clinic physician."
