Issue:

A Nurse Manager reports that a binder containing Patient Care Plans is missing from the unit. The binder contained care plans on approximately 24 inpatients. The care plans contained the patients' full name, diagnosis, nurse comments and discharge plans. Update: 01/07/13:All 24 patients will be offered credit protection services.

Outcome:

Nursing Leadership discontinued use of the hardcopy version of the care plan binders on the medical unit. Nursing staff have been instructed to use the electronic version of the Accucare system. Nursing staff were issued read only access to Accucare software system to review individual care plans. The printing capability was deactivated and users cannot print individual care plans. A Board of Investigation (BOI) is in process and the due date for the final report is March 29. Upon review of the final report and the recommendations, this ticket will be amended to reflect additional actions taken.
